FREQUENTLY ASKED QUESTIONS


I am a Permanente Physician, how do I access my CME transcripts?
  There are 7, easy-to-follow steps:
1) Go to MD Web Portal
2) Click on the My Personal Data tab
3) Log In (if you are a first time user, follow the onscreen instructions)
4) Click on the My Personal Data tab again
5) Do not log in
6) The box above the log-in box reads Physician Education - Click Here
7) Click on the My Personal Data tab.
How do I get copies of my certificates?
  Certificates are issued for each symposia you attend. We request that you keep your certificates somewhere safe and accessible. If you want to verify you attended a certain symposia, we can verbally inform you. If you need a reprint of a certificate, please do the following: For Professional Education symposia, please send a request via email to Amanda.C.Schwartz@kp.org or fax, tie 338-5029, 626-564-5029. The request must include the name and date of the symposia and if you would like it mailed (indicate where) or faxed. Please note requests take approximately 2 weeks or sooner depending on our workload. Requests for all of my classes since 2002 will take longer than two weeks because your signature will need to be verified for every meeting you paid for. How do I register for a continuing education symposium?
  In order to register for a symposium, you must first create a profile on our website. Once your profile is created, you can register for any 'open' event listed on the calendar page.
How do I register more than one person for the same symposium?
  Each person is required to have their own profile. You can register them but they need to have their own email address and password to log in.
Are non-Kaiser Permanente employees welcome at the educational symposia?
  Absolutely! Create a profile and register today!
Can I register online if I want to pay by check?
  Unfortunately, the online registration system is only available to those using credit cards. If you need to pay by check, please mail the completed registration form and check to the address on the registration form. Likewise, if you want to pay by credit card, but are not comfortable entering it online, please fill out completely the registration form, being sure to include the credit card number, and mail it to the address on the registration form.
How do I register for the Kaiser Permanente rate if I am a contract physician?
  Registration for contract physicians will be handled on a case by case basis. Please contact Liz Vega-Nelson in the Physician Education Department for details.
